Important Health Coverage Tax Documents

This notice is for anyone who was eligible for health coverage at CRSI for the 2025 calendar year.

Due to new federal legislation designed to reduce paperwork and administrative burden, we are informing you of changes to how Form 1095-C will be distributed.

Previously, we were required to automatically mail paper copies of IRS Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) to all individuals.  These forms are used to document that you met the Minimum Essential Coverage requirements under the Affordable Care Act (ACA).

However, due to recent changes in federal law, we are now only required to furnish this form to you upon request.

How to Request Your Form 1095-C:

If you would like to receive a copy of your 2025 Form 1095-C, please submit a request by using any of the following methods:

  • Website/Portal: You may also log in to your Paycor Account, at https://www.paycor.com/, Go to Pay & Taxes, Click on Taxes and select the correct year. If you are no longer an employee at CRSI, you have access to your Paycor Account for 18 months after your employment has ended.

Please include your name, address, and the last four digits of your Social Security number in your request. CRSI will mail or email your Form 1095-C within 30 days of receiving your request.
