Home/Community Based Waiver (HCBW)/(Individual Options Waiver)

This is the largest expanding federal/state Medicaid service. Individuals with MR/DD who are Medicaid eligible may choose their supports, such as, staff, transportation and settings.

Supported Living Services

CRSI offers individuals choice as to where they may live in their own homes or with family, using staff supports that allow for individual choices and decision-making. As with all of CRSI's supports/programs, this is truly consumer-driven. This is a totally state funded program with eligibility based on requirements of each county board of MR/DD.

Residential Facility Waiver (RFW)

This funding system allows for Medicaid funds to be used for housing, utilities, food, clothing, transportation, personal care, etc. Most supports are provided in group settings.

Intermediate Care Facility/Mentally Retarded (ICF/MR)

CRSI offers individuals to live in Intermediate Care Facilities for the Mentally Retarded. This setting offers high quality residential living in a shared setting, where individuals are supported with medical, transportation, program and other intensive services available to meet the specialized health needs of individuals with MR/DD.

CAFS (Community Alternative Funding System)

CAFS Mission Statement: The mission of the Ohio Department of Mental Retardation & Developmental Disabilities is to ensure the availability of programs, services, and supports that assist individuals in living the life they choose; to promote their health and safety; and to assist and support the families of these individuals in achieving these goals.

Eligible Individuals

Certified habilitation centers may seek reimbursement through CAFS for covered services that are provided either directly or through contractual agreements to individuals who:

  • Have a developmental disability(ies)
  • Have a developmental delay(s)
  • Are at risk (infants/children birth through 5 years of age)
  • Have a certified need

Effective July 1, 2001 CRSI was certified as a Habilitation Center to provide the following CAFS services: Service Coordination and Active Treatment.

As of October 16, 2001 CRSI was certified for the following additional CAFS services: Nursing, Delegated Nursing, Speech/Audiology, Psychology, Occupational Therapy, Physical Therapy, Social Work/Counseling, Nutrition and Transportation Services.

Quality Assurance

The goal of the Quality Assurance Department at CRSI is to maintain the agency's high level of quality care in diverse systems/models, counties, and consumer populations. Activities of the office include program evaluation, staff training, assistance with service delivery, ongoing monitoring of services, including utilization analysis and compliance to all appropriate rules and statutes that govern our services, consultation, and investigating, developing, and implementing innovative service delivery systems that provide greater support options and follow a pro-active consumer-driven philosophy of care.

A prime example of CRSI's proactive, consumer-driven services is our Consumer Advocacy Department, staffed by a full time Consumer Advocate who is an individual with a disability familiar with available supports and services. Consumer Satisfaction Surveys are performed by this department to assess the satisfaction of services provided by CRSI. The Consumer Advocacy staff works closely with the QA Department to ensure that CRSI follows up with any stated concerns and requests resulting from consumer satisfaction surveys. A follow-up quality of life survey is being developed to aid in assessing consumer satisfaction and providing input for the agency on areas our services should focus.

Vocational Services

The Vocational Services Department is staffed with two full time vocational rehabilitation employment services specialists, with a pool of part time job coaches.

The department operates under the requirements set forth by the Ohio Rehabilitation Service Commission and the CARF Accreditation Commission for Employee Development and Community Employment programs.

  • Career Exploration

    This service is provided to consumers that are unsure of the vocation that they wish to pursue. Through specialized testing, job shadowing and community based work assessments, consumers are able to explore and better determine their goals.

  • Job Development and Placement

    A specifically trained Job Developer assists the consumer in obtaining gainful employment in the field of their choice. This is accomplished through job seeking skills training, resume writing, assistance filling out applications and talking with potential employers.

  • Job Coaching

    Job coaching services are provided for consumers once employment is secured. The Job Coach provides individualized training, documentation, evaluation and advocacy for the consumer in the natural work setting. Job coaches remain at the work site until the quality and quantity of work required by the employer is attained.

Juvenile Employment Training Service (JETS)

The overall goal of the program is to reduce repeat offenders, instill responsibility to pay restitution and court costs, and establish a supportive learning environment that addresses the unique needs of youth offenders.

The objective is to create a "center of hope" that will offer alternatives for youth offenders and the opportunity to become good citizens with employable skills.

The program emphasis is on building good citizens through assistance with educational goals, employment training and job placement.

Family Coaching

A service provided to a family that is experiencing a variety of difficulties that may be threatening their ability to remain intact. In an effort to support the family and allow them to remain a unit, a coach is assigned to assess their strengths, provide support, encouragement and build on their current skills or teach the family new skills, so that after a brief involvement, the family can function successfully on their own.

Behavior Support Services

Behavior Assessment/Behavior Support Planning/Behavior Support Training/Consultation and Support: These service are provided to persons with MR/DD to assist them in accessing the effectiveness of needed services, receiving and making adaptations to appropriate services, skill building for staff and family members, coordinating and monitoring the services and supports a person may receive and handling crisis situations.

Delegated Nursing

The in service and oversight by a licensed nurse to unlicensed workers who are administering oral and or topical medications in the I/O or Supported Living environments.

Management Services

CRSI provides agency management services in both the program and fiscal areas. These services are contracted by other agencies for CRSI to provide a specific service such as fiscal oversight, management of direct service programs, ancillary services, etc. as stated in a signed agreement.
