Inc. recognizes the individuality of each consumer. Therefore, personal
choice remains the most important consideration and determines all
supports which surround the life of the consumer. Service supports
range from assistance with daily living skills to social/recreational
experiences and advocacy for the rights of individuals with disabilities.
Trained staff are prepared to provide services in Medicaid certified
ICF/MR funded homes, in addition to individualized support in either
a Supported Living or Community Based Waiver environment. |

Often there are many questions about how to access MR/DD services. This brochure serves as a brief outline of where to go and what to expect. |
Residential Options
Home/Community Based Waiver (HCBW)/(Individual
Options Waiver)
This is the largest expanding federal/state Medicaid service. Individuals
with MR/DD who are Medicaid eligible may choose their supports, such as,
staff, transportation and settings.
Supported Living Services
CRSI offers individuals choices as to where they may live in their own
homes or with family, using staff supports that allow for individual choices
and decision-making. As with all of CRSI's supports/programs, this is
truly consumer-driven. This is a totally state funded program with eligibility
based on requirements of each county board of MR/DD.
Intermediate Care Facility/Mentally Retarded (ICF/MR)
CRSI offers individuals to live in Intermediate Care Facilities for the
Mentally Retarded. This setting offers high quality residential living
in a shared setting, where individuals are supported with medical, transportation,
program and other intensive services available to meet the specialized
health needs of individuals with MR/DD.

Quality Assurance
The goal of the Quality Assurance Department at CRSI is to maintain the
agency's high level of quality care in diverse systems/models, counties,
and consumer populations. Activities of the office include program evaluation,
staff training, assistance with service delivery, ongoing monitoring of
services, including utilization analysis and compliance to all appropriate
rules and statutes that govern our services, consultation, and investigating,
developing, and implementing innovative service delivery systems that
provide greater support options and follow a pro-active consumer-driven
philosophy of care.

Consumer Advocacy

A
prime example of CRSI's proactive, consumer-driven services is our
Consumer Advocacy Department, staffed by a full time Consumer Advocate
who is an individual with a disability familiar with available supports
and services. Consumer Satisfaction Surveys are performed by this
department to assess the satisfaction of services provided by CRSI.
The Consumer Advocacy staff works closely with the QA Department
to ensure that CRSI follows up with any stated concerns and requests
resulting from consumer satisfaction surveys. A follow-up quality
of life survey is being developed to aid in assessing consumer satisfaction
and providing input for the agency on areas our services should
focus.
The Consumer Advocacy Director is responsible for the oversight
of the MUI process thus ensuring the health and safety of all consumers
served. |

Vocational Services

The Vocational Services
Department is staffed with two full time vocational rehabilitation
employment services specialists, with a pool of part time job coaches.
The department operates under the requirements set forth by the
Ohio Rehabilitation Service Commission and the CARF Accreditation
Commission for Employee Development and Community Employment programs.
CRSI provides a youth program (ages 15-19) in partnership with
the Ohio Developmental Disabilities Council, for teens with disabilities
who are transitioning from high school to the workforce. Transition

Career Exploration
This service is provided to consumers that are unsure of the vocation
that they wish to pursue. Through specialized testing, job shadowing
and community based work assessments, consumers are able to explore
and better determine their goals.
Job Development and Placement
A specifically trained Job Developer assists the consumer in obtaining
gainful employment in the field of their choice. This is accomplished
through job seeking skills training, resume writing, assistance
filling out applications and talking with potential employers.
Job Coaching
Job coaching services are provided for consumers once employment
is secured. The Job Coach provides individualized training, documentation,
evaluation and advocacy for the consumer in the natural work setting.
Job coaches remain at the work site until the quality and quantity
of work required by the employer is attained.

Behavior Support Services

Behavior Assessment/Behavior
Support Planning/Behavior Support Training/Consultation and Support:
These service are provided to persons with MR/DD to assist them in
accessing the effectiveness of needed services, receiving and making
adaptations to appropriate services, skill building for staff and
family members, coordinating and monitoring the services and supports
a person may receive and handling crisis situations. |

Medication Administration
|
Medication Administration is provided by fully trained staff through in-service training and oversight by a licensed nurse to unlicensed personnel who are administering any medications in I/O or Supported Living environments.
This comprehensive training course is conducted by qualitfied and
experienced nursing staff to provide safe medication administration for
consumers according to rules set forth by the Ohio Department of
Mental Retardation and Developmental Disabilities. For details contact

Management Services
CRSI provides agency management services in both the program and fiscal
areas. These services are contracted by other agencies for CRSI to provide
a specific service such as fiscal oversight, management of direct service
programs, ancillary services, etc., as stated in a signed agreement.
Vocational
Habilitation and Adult Day Services

CRSI Adult Day Services is offered in select counties as a program of choice for individuals who want to be involved in recreational and/or vocational habilitation activities."
